Release and Waiver of Liability & Media Authorization

In consideration of the permission granted to me by RISE UP Cooperative (“Releasee”) to participate in events, workshops, and activities hosted or instructed by RISE UP Cooperative, and to allow the use of my image, likeness, voice, video, audio, photographs, or other materials (“My Participation”), I agree to the following:

1. Media Release & Authorization

I hereby grant RISE UP Cooperative, its Board of Directors, officers, employees, agents, and representatives the irrevocable right and permission to:

• Use, reproduce, publish, distribute, and display photographs, video, audio, or other media of My Participation in any form, including but not limited to web-based publications, print publications, organizational usage, and social media publications.

• Use my name, likeness, image, voice, performance, and/or biography in connection with such materials.

• Edit, copyright, and adapt such materials at their sole discretion.

I acknowledge that I will receive no monetary compensation for these rights and that RISE UP Cooperative has no obligation to use or publish such materials.

2. Waiver & Release of Liability

I hereby release and discharge RISE UP Cooperative, the State of Tennessee, its Board of Directors, officers, employees, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to My Participation, including but not limited to the use of media as described above.

I further agree to indemnify and hold harmless RISE UP Cooperative, its Board of Directors, officers, employees, and agents from any loss, liability, or expense (including reasonable attorney’s fees) arising from claims that My Participation, including statements, actions, or materials I provide, infringes or violates the rights of third parties.

3. Parking & Personal Property

I acknowledge that I am solely responsible for my vehicle and personal belongings during events. Any parking tickets, violations, damages, or losses are my responsibility. RISE UP Cooperative assumes no liability for parking arrangements or incidents.

4. Acknowledgment of Understanding

I understand and affirm that:

• This Release is binding under the laws of the State of Tennessee and any other jurisdiction in which my participation occurs.

• I am at least 18 years of age, OR I am the parent or legal guardian of a participant under 18 years of age and hereby consent to this Release on their behalf.

• I have read this Release in full, understand its terms, and sign it voluntarily and with full knowledge of its legal significance.
